What You Need to Know Before Your Visit
We look forward to seeing you for your retinal evaluation. Our physicians are board-certified and fellowship-trained retinal specialists. We are one of the only subspecialty groups in the region devoted exclusively to the treatment of retinal disorders. We seek to provide state-of-the-art care in a compassionate environment.
Insurance: We participate with many insurance carriers and Medicare. We suggest that you contact your insurance plan to confirm our participation with your plan.
Referrals: Some insurance plans require a referral to pay for services. If a referral is required, it is the patient's responsibility to obtain the necessary referral prior to the visit.
Prior to your appointment:
To expedite your check-in process for your appointment, please complete all questions and digital forms under Complete Registration. We also ask that you upload a copy of your current medication list using Upload Document option under myChart.
Your Appointment Day
Bring the following with you:
- Current medication list
- Photo ID
- Insurance card(s) - If you have a Medicare Advantage/Replacement plan, we will also need you to provide your government issued Medicare ID.
Additional information:
- Please arrive 15 minutes prior to your actual appointment time.
- Your eyes will be dilated; we recommend a driver. The effects of the dilation can last up to several hours. Please bring sunglasses.
- Do NOT wear contacts to this appointment, please wear glasses.
- Your visit may last 1½ to 2½ hours.
- Copayments are due at time of service. Please contact our Billing Department at 920-751-8666 option 9 if you have payment-related questions.
