Northeast Wisconsin Retina Associates participates with many insurance carriers and Medicare. We suggest that you contact your insurance provider to confirm our participation with your specific plan. We will gladly assist you with any necessary paperwork and other requirements relative to your insurance provider.
Please bring the following for each visit:
- Your current insurance card(s)
- If you have a Medicare Advantage/Replacement plan, we will also need you to provide your government issued Medicare ID
- Copayments, if required by your insurance payer
